    1.   Complete the digital form (PayPal Membership Form)
    shown in the box below and submit it electronically.  Use
    PayPal as your payment option.   Credit cards are
    accepted through PayPal.



                        


    2.   Print one of the paper forms (click on one of the Paper
    Membership Form boxes), complete the form off line, and
    mail it along with your check made out to SLADS to
    Shelley Brubaker at the address shown.   Be sure to
    provide all the information requested.
